My Seventh week Journal
       My seventh week in my OJT. This week my task is to update again the policy because I did not finished it last week. I found out that those policies need to change after 3 years , then they made a new policies needed to followed and be obeyed every department. I knew that policies is very important because if have a policies there have a base that they need to follow so every employees know there responsibilities doing and they working smoothly.
My Sixth week Journal
           My sixth week in my OJT. My task is to update the policy of every department. My supervisor is the point person when it comes to policy documentation. Everyday she receives a new policy, which she then she need to segregate. After that, the policy must be distributed to the nursing units. I am the one to re-arranging the policy, put the title given and the document code. Then when I am finished, I need to removed the superseded and canceled policy. The superseded means is the old policy then i changed to the newest one. there's a many policy given in the St. Lukes medical center that need to followed and to b obeyed. This is my new discovery with regards to their policy standards.

My Fifth Week Journal     

         My fifth week in my OJT. This week is auditing week. It is my first time to be with my supervisor to audit the discharge process of the patient in the hospital. Before discharging the patient, they have to ask questions and it will be answered by patients who will be discharged. I have known that, it is not necessarily needed to write the full name of the patient in the discharge form for their safety and security. Writing their initials is accepted. After that I went to the units and asked those people who are in charge for the preparation during the “Quality Day”

My Fourth Week Journal    

       My fourth week in OJT. After a long vacation, I am back to work again. My duties and responsibilities in this week were: arranging the files of my supervisor, because the files of my supervisor are mixed so I need to segregate all of it and keep all folders with their label. So that if ever my supervisor needs the files, it is easy to find it because there is a name or label. Again, segregate the policy and distribute it to the different units. Then I went to the different units to give the memo for the workshop which is a preparation for the so-called “Quality Day”.


My third Week Journal          

       This week is my third week in my OJT. My tasks were labeling or naming the folder for the document of my supervisor, sorting and filing of documents, and finally encoding and segregating the policy documents. After segregating the policy, I need to distribute the policy to the different units. Ask them to sign it confirming that they have agreed on the policy.

My Second week Journal

       During my second week OJT, I learned about policies of each cluster or unit area. St. Luke’s Medical Center has 32 unit area or 6 clusters in its nurses station. First, I segregate the policies, then after that I need to distribute it in the unit area. The policy papers must be signed the policies as proof that they have agreed on the policies. Policies must be strictly followed or implemented.I also experienced auditing or computing the Multi-disciplinary assessment database (MIAD). During my first attempt, it was hard to do since I don’t have any background in handling such database. As I continue using it, I think it is easy now for me to use it. I also encountered confidential documents called Safety learning event analysis (SLEA). It is about what happens to a patient inside the hospital or during a patient’s stay at the hospital. The documents is treated as strictly confidential.As a summary, this week I have learn about the confidentiality of documents.

My First week Journal               My first week experience in my On the Job Training (OJT) was I'm alone because in my department have a two department. The department of Clinical Advancement (DCA) and the Nursing Service Office (NSO). I assigned in the NSO department, that time the staff of NSO office is in the seminar or STRAT plan, so for the mean time I will assigned in NSO office because there's no staff in that office. So I assigned in the front desk. My duties and responsibilities are answering incoming calls, receiving files, and sometimes entertaining visitors. Then labeling the folders and arranging all files. My task in that week was commonly but its okay maybe in the other weeks I have a new learning's.
